Disability Evaluation Squitieri Scholarship

  • Applicants must complete and submit the following documents. Incomplete applications will not be reviewed.
    • Scholarship application (located below)
    • Personal Statement - a personal statement why you suspect you have a learning disability or ADHD. Include your attempts to seek funding from other sources e.g. health insurance.
    • A copy of your financial aid award letter (can be found on ONE.UF)
    If your application is approved, you will be contacted by the DRC and instructed on how to connect with your evaluator to schedule your evaluation appointments. Please tentatively plan on having two appointments for actual testing and a follow up appointment to discuss your evaluation results. NOTE: This can be a lengthy process. It is not guaranteed that you will be able to receive accommodations, if eligible, within the same semester you are being evaluated. Once you have received your evaluation results, please contact the DRC at accommodations@ufsa.ufl.edu.

  • Accepted file types: pdf, doc, docx, txt, rtf, Max. file size: 32 MB.
  • If my scholarship application is accepted, I authorize the Disability Resource Center to release information regarding my application to the evaluator that will be conducting the evaluation. This may include: a copy of my scholarship application, a copy of my transcript, and the results of any screeners that may have been administered by the DRC. I agree to attend the scheduled appointments with my evaluator and will give adequate notice in the event I need to re-schedule. Missed appointments could result in the revocation of the scholarship.
